Valley Medi Van has requested a Vehicle for Hire Permit to <br />operate a non-emergency medical transportation business in this City. The applicant reports being <br />licensed in the cities of Moreno Valley and Perris. Inquiries by the City of Riverside indicate no negative <br />reports in either city. The application accompanies this report as Attachment 1. <br /> <br />Ms. Cabe reports that she has sixteen years of experience in the non-emergency medical transportation <br />business. The application submitted this year indicates that there would be three van drivers: Mr. Marvin <br />Sims, Ms. Elizabeth Gago and Ms. Somchit Cabe. The applicant now reports that the only driver will be <br />Ms. Somchit Cabe although there are two transport vans. <br /> <br />The City has had previous contact with this applicant: <br /> <br />1. A previous application from this applicant was denied by the City Council without prejudice on January <br /> 6, 1987. <br /> <br />A letter from the City was sent to the applicant on June 2, 1994 to cease and desist soliciting a <br />transportation business in this City at Riverside General Hospital after the City received a complaint <br />(Exhibit 1). The applicant was invited to apply for a Vehicle for Hire Permit, but an application was not <br />forthcoming. <br /> <br />On July 24, 1996, the City again sent a letter to the applicant to immediately cease and desist soliciting <br />and conducting a business in the City of Riverside (Exhibit 2). This letter resulted from two complaints <br />filed with the City. <br /> <br />On April 15, 1999, the State Department of Health Services wrote to the applicant and reported that <br />their on-site visit of the applicant's business was not as represented in her certification and therefore <br />the applicant was out of compliance with State regulations to be a Medi-Cal provider (Exhibit 3). On <br />October 7, 1999 City staff confirmed with a representative of the State Department of Health Services <br />that this applicant continued to be in violation of the cited sections of State regulations (see April 15, <br />1999 letter). <br /> <br />On October 29, 1999, the City again wrote to the applicant regarding other deficiencies in the <br />applicant's application for a Vehicle for Hire Permit (Exhibit 4). <br /> <br /> 10-1 <br /> <br /> <br />
