P 1, Y f. <br /> RIVERSIDE PUBLIC UTILITIES <br /> P U B L I C U T I L I T I E S Board Memorandum <br /> BOARD OF PUBLIC UTILITIES DATE: November 18, 2011 <br /> ITEM NO: 5 <br /> SUBJECT: CHANGE ORDER NO. 1 TO PURCHASE ORDER NO. 114146 CONDUIT AND <br /> STRUCTURE INSTALLATION FOR CASA FLORES COMPLEX AND MAGNOLIA <br /> CONVALESCENT HOSPITAL AT 8123 MAGNOLIA AVENUE - WORK ORDER NO. <br /> 1104644 <br /> ISSUE: <br /> Board of Public Utilities approval is requested for Change Order No. 1 to Purchase Order No, 114146, in <br /> the amount of $20,786.81 for additional trenching, conduit and structure relocations due to field <br /> conditions and the removal and replacement of an inaccessible buried transformer enclosure. <br /> RECOMMENDATION: <br /> That the Board of Public Utilities approve Change Order No. 1 to Purchase Order No. 114146, in the <br /> amount of $20,786.81. <br /> BACKGROUND: <br /> Work Order No. 1104644 for conduit and structure installation for Casa Flores complex and Magnolia <br /> Convalescent Hospital at 8123 Magnolia Avenue was approved by the Board of Public Utilities on <br /> December 17, 2010 for an estimated capital expenditure of $147,000. The conduit and structure portion <br /> of the project was issued to the Contractors' Panel for Energy Delivery work on January 7, 2011. Bids <br /> were opened on January 18, 2011 and Southern Contracting Co., of San Marcos, California, was the <br /> lowest responsive bidder in the amount of $54,543.28. <br /> Change Order No. 1, in the amount of $20,786.81 is requested due to: <br /> • Existing AT&T facilities were in conflict with new structures and thus had to be relocated. <br /> • Partial primary riser pole trenching had to be reinstalled due to conflicts in the street. <br /> • Remove and reinstall existing handicap concrete chair ramp impacted by trenching. <br /> • Additional concrete encasement and adding of dye to identify RPU conduits with only 24" of cover. <br /> • Removal of an existing buried transformer enclosure that was inaccessible due to its location <br /> under a stairwell and replacement with a new traffic rated buried transformer enclosure. <br /> • Additional paving and traffic control was required due to field conditions. <br /> Board authorization is required as the cumulative change order amount exceeds 10% of the original <br /> purchase order amount as summarized in the following table: <br /> Percent <br /> Descri tion Amount Change <br /> Original purchase order $54,543.28 <br /> Change order No. 1 $20,785.81 38.11% <br /> Total $75,330.09 38.11% <br /> The need for the extra work is due to unknown and changing field conditions some of which is on private <br /> property.
